CHRONIC HOMELESSNESS DEFINED:
an individual who has been continuously homeless (sleeping in a place not meant for human habitation or in a homeless emergency shelter) for at least 1 year or experienced at least 4 episodes of homelessness in the last 3 years that totaled at least 1 year and has a disabling condition.

Housing First is an approach to quickly and successfully connect individuals and families experiencing homelessness to permanent housing without preconditions and barriers to entry, such as sobriety, treatment or service participation requirements.
Above all, we provide an individual relationship and safe space for our residents to not just feel like program participants, but part of the community.
OUR CASE MANAGEMENT APPROACH »
Our on-site case management team is readily available and easily accessible for Flenniken residents. We walk alongside each of our residents to help them create an individual plan that will improve their quality of life. We provide feedback about areas of concern or improvement, information about resources that may be beneficial to them and problem solving skills for them to better maintain housing and a healthy lifestyle.
